The cleanup function of the mmap_list, now removes list elements that their index does not match their hash. This gives a real boost! I suspect that this is because of avoiding duplicate entries.

More improvements on the mmap caching code. Removed the MMAP_LIST_USE_MAX. Now the upper limit is the size of the mmap list.
